About The Therapist

I am Jordan Williams, a Licensed Associate Professional Counselor (APC 000579) practicing in the State of Pennsylvania under the supervision of Jacob Glickman, Psy.D., LPC, NCC. I hold a Bachelor’s degree in Psychology from Texas Christian University, a Master’s degree in Clinical Mental Health Counseling  from Villanova University, and I am currently pursuing a Ph.D. in Counselor Education at The George Washington University. I am also recognized as a National Certified Counselor by the National Board of

Certified Counselors.

My areas of specialty center on providing affirming care for BIPOC and Queer communities, with a focus on complex trauma, intersectionality, intergenerational trauma, and identity development. I have extensive training and experience working with Transgender and Gender Non-Conforming (TGNC) individuals, including offering gender-affirming care and letters of support. I am certified in Liberation Focused

Therapy and approach counseling through a holistic, trauma-informed perspective. Guided by a social justice framework, I strive to create a compassionate, inclusive, and culturally responsive space that supports healing and growth. My goal is to empower clients by honoring their strengths, walking

alongside them through challenges, and fostering resilience and self-awareness.

General Information

The therapeutic relationship is unique in that it is a highly personal and at the same time, a contractual agreement. Given this, it is important for us to reach a clear understanding about how our relationship will

work, and what each of us can expect. This consent will provide a clear framework for our work together. Feel free to discuss any of this with me. Please read and indicate that you have reviewed this information and agree to it by filling in the checkbox at the end of this document.

The Therapeutic Process

You have taken a very positive step by deciding to seek therapy. The outcome of your treatment depends largely on your willingness to engage in this process, which may, at times, result in considerable discomfort. Remembering unpleasant events and becoming aware of feelings attached to those events can bring on strong feelings of anger, depression, anxiety, etc. There are no miracle cures. I cannot promise that your behavior or circumstance will change. I can promise to support you and do my very best to understand you and repeating patterns, as well as to help you clarify what it is that you want for yourself.

Confidentiality

Your privacy is very important to me. All information shared in therapy will remain confidential, except in circumstances where disclosure is required by law or professional ethics. These exceptions include:

- If you express intent to harm yourself or others.

- If there is suspected abuse or neglect of a child, elder, or vulnerable adult.

- If I am ordered by a court of law to release information.- If treatment is mandated by a court or legal proceeding.

- If I consult with other professionals for the purpose of improving your care (in which case identifying details will be withheld).

Client Rights and Responsibilities

As a client, you have the right to be treated with dignity and respect. You have the right to participate actively in setting the goals of your treatment, to ask questions about the therapeutic process, to ask for your notes, and to refuse or discontinue services at any time. You also have the right to request referrals to another provider if you believe that would be in your best interest. As a client, you are responsible for attending scheduled sessions and for providing notice if you need to cancel an appointment. You are also responsible for being open and honest in therapy and for engaging as actively as you are able in the therapeutic process.

Appointments & Fees

The standard meeting time for individual sessions is generally 50-53 minutes. Unfortunately, extended 90-minute sessions are not available at this time. Sessions are conducted via telehealth. Telehealth sessions are conducted via HIPAA-compliant secure video platform. In the event of technical difficulty or outage of the primary secure video platform, sessions may be conducted via a secondary HIPAA-compliant secure video platform identified at that time. The fees for each service are outlined on the Rates and Payment page of the practice website. These fees apply to all clients unless outlined elsewhere in a reduced fee, third-party payer, or scholarship agreement. Payment can be made via credit/debit card or ACH bank transfer. Please note that processing fees may be assessed for any electronic payments based on payment

processor. You agree that you will not dispute charges (“charge back”) for sessions that you have received or appointments that you have missed or late cancelled according to this or any other practice policy; you agree that doing so constitutes fraud and is grounds for immediate suspension and termination of services. An inconvenience fee of $35 per disputed charge will be added to your account to cover costs associated with addressing the matter. You agree to make payment for sessions utilizing electronic methods for which you are the owner and/or an authorized user; you agree to provide written proof of ownership and/or authorization if necessary, and to be held liable for any and all charges associated with your use of such a method, including any fees resulting disputes initiated by another authorized user. Failure to make payment for two consecutive weeks may result in suspension and/or termination of services.

Superbills

The practice is out of network. Clients connected to insurance plans who wish to submit for out-of-network billing will be provided with a monthly receipt ("superbill") for out-of-pockets payments. Clients who choose to pursue insurance reimbursement should be advised that their insurance company may request a formal diagnosis and/or treatment records in order to approve payment. Due to this being a non-pathological practice, we will explore the cultural considerations of the diagnostic process together before anything is documented.

Lateness, Missed Sessions & Rescheduling

The grace period for lateness is 10 minutes. If you are late beyond 5 minutes, I may not be available to see you. If you are running late or suspect that you will be late, you must call, send an email, or send a secure message through the Client Portal before your appointment time to confirm that you can still be seen. If you are late beyond 10 minutes and do not reach out, you may be considered a no-show. If you are late for a session, you will lose some of that session time. Sessions will not be extended beyond the scheduled time slot. All clients are given a standing appointment at the start of treatment; the standing appointment date and time will remain in place and is never offered to another client nor changed unless you are not seen for two consecutive weeks, or there is prior agreement made. Failing to attend your standing appointment twice in a calendar month may result in your being removed from the standing appointment schedule and the standing appointment time may be offered to another client or an individual on the waiting list. If this occurs, you will be asked to schedule on a week-to-week basis. This practice has a 24- hour cancellation policy for follow-up appointments. This means that you will still

be responsible for the full session fee if you cancel with less than 24 hours notice or fail to show for your scheduled appointment time as it cannot be offered to anyone else. Payment must be received before you are able to schedule another session.

Telehealth Services

The practice owner, Jordan Williams, LAPC, NCC currently holds an Associate license to practice in the state of Pennsylvania (APC000579). This means at the time of service, you must be physically located in one of the states in which your therapist is authorized to practice in order to be seen for your appointment; if you are physically located elsewhere at time of service, you cannot be seen.

Social Media

Due to the importance of your confidentiality and the importance of minimizing dual relationships, I do not accept friend or contact requests from current or former clients on any social networking site (Tiktok,

Instagram, LinkedIn, Twitter, etc). I believe that adding clients as friends or contacts on these sites can compromise your confidentiality and our respective privacy. It may also blur the boundaries of our therapeutic relationship. If you have questions about this, please bring them up when we meet and we can talk more about it.

Interactions Outside the Therapeutic Space

As a member of certain overlapping communities, you may run into me in public spaces, at social gatherings, or at other events in the community. I will never acknowledge you first in order to help you avoid other people’s questions about how you know me, and will only make brief, light conversation if you approach me first. I will never bring up therapeutic details in public and clients are asked to abide by the same boundary. If you anticipate or have concerns that you may run into me at a specific gathering in the community, you are encouraged to discuss this with me ahead of time to assess and address any privacy, confidentiality, or ethical concerns.

Termination

Ending relationships can be difficult. Therefore, it is important to have a termination process in order to achieve some closure. The appropriate length of the termination depends on the length and intensity of the treatment. The 'ideal' termination process is mutually agreed upon and collaborative. In certain circumstances, I may terminate treatment after appropriate discussion with you and a termination process if I determine that the psychotherapy is not being effectively used or if you are in default on payment. I will not terminate the therapeutic relationship without first attempting to discuss and explore the reasons and purpose of terminating. If therapy is terminated for any reason or you request another therapist, I will provide you with a list of emergency services and/or qualified psychotherapists to treat you. Should you fail to attend an appointment for two consecutive weeks, unless other arrangements have been made in advance, and fail to respond appropriately to attempts to re-engage you in care, for legal and ethical reasons, I must consider the professional relationship discontinued.

Emergency and Crisis Situations

I may not be immediately available between sessions. If you are experiencing a life-threatening emergency, please call 988 or go to the nearest emergency room. You may also use your personal safety plan, if applicable. For non-emergency situations, please contact me through the Client Portal and I will respond as soon as possible, typically within one business day.

Code of Ethics

I adhere to the American Counseling Association (ACA) Code of Ethics, the National Board of Certified Counselors (NBCC) Code of Ethics, and the regulations governing my license. If you ever have concerns about the quality of care you receive, I encourage you to raise them with me so that we can address them together. If concerns are not resolved, you may contact the appropriate licensing or credentialing boards.
